(PCAP) Primary Care Asthma Program
This program is in connection with Ontario Lung Association and Asthma Research Group. It is currently provided on a part-time basis to rostered patients of The Harrow Health Centre. It is an opportunity for patients to speak directly with Certified Respiratory Educators and work along with them to develop a treatment plan appropriate to their needs.
CCAC – Community Care Access Centre
This service will assess and assist to navigate patients through the wide range of social and community services available to support our patients. CCAC staff is on site on a full-time basis.
This program includes risk screening, treatment, patient and family education. We also provide symptom management and complication prevention strategies. Referrals are made and monitored according to best practice guidelines. These guidelines recommend the best care for our patients and at times are modified based on new research.
Primary Care Medical Clinic
The goal of this program is to provide patients and care givers timely access to comprehensive and high quality dementia care. An inter-disciplinary team, specially trained in caring for people living with dementia, works with patients and care givers to complete comprehensive assessments, identify concerns and resources, and create a plan for care.
A Certified Dietitian is available, on a part-time basis, to discuss nutritional needs and assist in promoting a healthier community. If you are interested in speaking with our dietitian please call 519-738-2000
This program includes an assessment of your current immunization status. We will update your immunizations and records. An annual flu shot and other clinics are offered as recommended by the Public Health Unit.
Mental Health Program
The Harrow Family Health Team also has a Registered Social Worker available to counsel individuals for their specific needs. If you are interested in speaking with her please contact your primary provider.
Ontario Telemedicine Network
Telemedicine uses telecommunication technology to provide clinical health care in Ontario at a distance. This technology provides access to specialists and services that often would not be available consistently in our area.
Our on-staff pharmacist may assist in a comprehensive medication review and medication reconciliation to promote better understanding of your medical care as requested by your doctor or nurse practitioner. The Pharmacist also plays a part in our Diabetes Education and our Smoking Cessation program (STOP). STOP is a treatment program for Ontario patients who wish to stop smoking. Our RPN and Pharmacist are certified in this program to assist you with free Nicotine Replacement Therapy as well as counselling and support for program success.
This includes assessment of your health needs and making an appropriate plan for you. The Centre maintains a current, comprehensive knowledge of programs and services in Windsor and Essex County. We will recommend and assist you to connect with other health and service providers.
Quality Improvement and Innovative Partnership
QIP is a quality improvement organization funded by the Ministry of Health and Long Term-Care. The goal of this program is to advance the development of a high performing primary healthcare system. Through this initiative we will be looking at ways to improve access to our patients. We are continually evaluating the way the clinic functions and ways to improve it.
